Adding Youtube videos to your forms is extremely easy:

1. In the form editor, click 'Add Field'.
2. In the field type selector, pick 'Youtube Video'. It'll be in the Informative Fields section.
3. Copy-paste the Youtube video's URL into the light-yellow box.
   

Done! You should now see the Youtube video in your form. Try playing it!
